حاليا من أشهر عمليات السمنة لأسباب واضحة.. بدأت كجزء اول من عملية اخري من أنواع تحويل المسار- تحويل الاثني عشر مع التكميم- ثم بدأت في حوالي عام 2002 كعملية مستقلة علي يد الجراح الأمريكي ميشيل جانييه.
استخدام المنظار مع بساطة المبدأ الجراحي ادي الي انتشارها السريع بين أوساط الجراحين.. ومع نسب النجاح العالية مع عدم التقيد بالمكملات الغذائية لفترات طويلة بعد الجراحة زادت شعبيتها بين المرضي أيضا..
تعتمد فكرتها علي استئصال الجزء الأكبر من حجم المعدة مع تصميم معدة جديدة ضيقة (قد يصل حجم المعدة في الطبيعي الي 2500 ملل وبعد العملية يكون الحجم المتوقع 200 ملل في المتوسط) مما قد يقلل حجم الوجبات بشكل كبير جدا.الجزء المستأصل أيضا يشمل الجزء المسئول عن افراز هرمون الجوع-هرمون الغريلين- مما يقلل من الإحساس بالجوع بشكل كبير جدا بعد العملية مما يساعد علي تقبل المريض بكل سهولة لتغيير نمط حياته والتعود علي نظام الحياة الصحي. واجهت الجراحين المشكلة الأكبر في التعامل مع المضاعفات الخطيرة بعد العملية ( النزيف والتسريب) واللتي كانت تصل في بعض المراكز ال 10% ولكن مع تطور تكنولوجيا الالات ومع تراكم الخبرات الجراحية فقد انخفضت هذه النسبة لتصل الي اقل من 1% في المراكز ذات الخبرة الجيدة

مميزاتها:

• يمكن اجرائها في الاعمار الصغيرة (سجلت في العالم حالات في الأطفال بأعمار سنتين )
• سهولة التكنيك الجراحي
• عدم الالتزام بمكملات غذائية لفترات طويلة.
• معدل النجاح العالي.

عيوبها:

• غير مناسبة لمرضي ارتجاع المرئ.
• رجوع الوزن علي المدي الطويل لبعض المرضي وخاصة غير الملتزمين بالتعليمات.
• لن تكون الخيار الأول لمرضي السكر او في الاعمار المتقدمة

الحقيقة الصادمة انه من المفترض ان تكون العملية معيارية بحيث ينصب اهتمام الجراح علي تصميم الجزء المتبقي من المعدة (والمفترض ان يكون ثابت لكل المرضي بالمناسبة) ويستأصل الجزء الباقي واللذي يختلف من مريض لاخر اذن فالجزء المستأصل يختلف بحسب اختلاف المريض (معداتنا ليست بنفس الحجم) وليس علي هوي الجراح
